Medicaid Service Coordination (MSC)

MSC is a Medicaid State Plan, designed to assist mentally retarded and developmentally disabled individuals in gaining access to necessary services and supports appropriate to their needs. An MSC Service Coordinator works closely with each consumer to ensure that such services and supports are obtained.

Age group served – 3 years and up

In-Home Residential Habilitation (Res Hab)

Res Hab provides assistance to mentally retarded and developmentally disabled individuals to help them live as independently as possible. The program’s goal is to enable each consumer to reside in a non-institutional setting. Habilitation services are provided at home.

Upon admission, a trained Habilitation Specialist assists the consumer in acquiring, maintaining, and/or improving independent living skills, such as grooming, shopping, budgeting, meal preparation, social and adaptive skills.

Age group served- 3 years and up

Day Habilitation (Day Hab)

Day Hab helps mentally retarded and developmentally disabled individuals achieve satisfying and rewarding connections and relationships within the community. Out of home training at volunteer sites allow each participant to reach maximum potential.
